Chelsea are one win away from lifting the Premier League trophy for the fifth time, and the English top division trophy for the sixth time.

Did you know? Chelsea won their first title in their 50th year of competitive football!

The Blues first trophy winning season was 1954/55. They won the title with a game to spare by beating Sheffield Wednesday 3-0 at Stamford Bridge. At that time, England's top league was called the First Division.

Chelsea had to wait another 50 years before their next big success.

Did you know? Chelsea's top scorer in history Frank Lampard scored two goals in their first Premier League title-winning match.

In the 2004/05 season, The Blues beat Bolton 2-0 to secure their first Premier League title.

Manager Jose Mourinho, in his first season with the club, made sure his side maintained their great form to the end of the season. The Blues won two of their last three matches and drew the other, even though they'd already won the title.

In Mourinho's next season, Chelsea secured a historic second Premier League title in two years!

Did you know? Chelsea won the 2005/06 title by beating main challengers Manchester United 3-0!

The next time Chelsea won the trophy, in 2009/10, they again beat Manchester United to the title, but this time it was much closer.

The Blues won on the last day of the season, but the result will be remembered for another reason...

Did you know? Chelsea recorded their biggest ever win in their final match of the 2009/10 season, beating Wigan 8-0 and lifting the Premier League trophy in the process!

The Blues won their most recent trophy in the 2014/15 season, two years ago. Eden Hazard scored the only goal as the Blues beat Crystal Palace 1-0 with three games to spare.
